Prescription Medication Returns
Due to state and federal regulations, dispensed prescription medications are generally not eligible for return once shipped.

If a provider determines that you are not eligible for treatment after review, any applicable refund will be handled according to the terms presented during the applicable intake or checkout flow. If you have an issue with an order, charge, or fulfillment step, contact support and we will help route your request appropriately.
Non-Prescription Purchases
If FormBlends offers eligible non-prescription merchandise or promotional items directly, any return instructions and refund eligibility for those items will be provided at the time of purchase.
Damaged or Defective Shipments
